The CTA Voluntary Life Insurance Plan* offers members a flexible, level benefit plan that will help members purchase a new life insurance plan or supplement the coverage they may have.
- Premiums start as low as $1.50 a month.
- Coverage options range from $25,000 to $400,000.
- Spouse and dependent coverage options are available.
- Advanced payments are available for critical illness.
- Participants in the CTA Voluntary Life Insurance Program are automatically eligible for the CTA Health Information and Wellness Program at no additional cost.
*Effective September 1, 1998, the voluntary life insurance plan changed from a decreasing term to a level term benefit design. Members who enrolled in the CTA Decreasing Term Life Insurance Plan prior to September 1, 1998 can transfer their coverage to the new CTA Level Term Life Insurance Plan during an open window each year. Watch for direct correspondence from UnumProvident or contact them at 1-800-282-4049.

How to Obtain this Benefit |

Life Insurance is available to all eligible CTA members.
Become a member of CTA by:
- Requesting a Membership Application at:

To enroll in the Voluntary Life Insurance Plan:
- Request a Life Insurance Plan Brochure from the Member Benefits Order Form
- Call UnumProvident's CTA Customer Service Unit at:
(800) 282-4049
- Visit www.unumprovident.com/enroll/cta
CTA members are encouraged to apply during Open Enrollment Periods. However, members can apply for coverage at any time.
- During an Open Enrollment, if you meet the eligibility requirements of the plan, your application for up to $200,000 of member coverage will be automatically accepted, and you will not have to answer the health questions on the application.
- If you do not apply during Open Enrollment your application will be subject to health evidence underwriting and insurance company approval.
There are two Open Enrollment opportunities offered by the plan:
- For new teachers, Open Enrollment is during the first 120 days of their employment.
- For all chapter members, Open Enrollment occurs if your Chapter conducts a successful enrollment campaign and the necessary participation requirements are met.
Once accepted into the Plan, and coverage options are selected, premiums are automatically deducted from your paycheck.
