Death and Dismemberment

---- Overview ----
The CTA Death & Dismemberment Plan is an automatic, no-cost benefit available to eligible CTA members.  The Plan currently pays a one-time Death Benefit of up to $2,000, and a one-time Accidental Death and Accidental Dismemberment Benefit of up to $10,000.  The benefit increases to a $50,000 payment should the death or dismemberment result from an accident or assault which occurs while you are actively engaged in your occupation or while you are acting in the capacity of an Association Leader.
  1. Death Benefit
    Upon the Death of a Participant, the benefit payable is an amount equal to the number of Years of Continuous Membership multiplied by $200 per year, up to a maximum benefit of $2,000.
  2. Accidental Death Benefit
    If the Death of a Participant is caused by an Accident or Assault and occurs within 90 days of the Accident or Assault, the benefit payable is an amount equal to the number of Years of Continuous Membership multiplied by $1,000 per year, up to a maximum benefit of $10,000.  This benefit is in addition to the Death Benefit.  This benefit will not be paid if the Accidental Dismemberment Benefit has been paid as a result of the same Accident or Assault.
  3. Accidental Dismemberment Benefit
    In the event the Dismemberment of a Participant is caused by an Accident or an Assault and occurs within 90 days of the Accident or Assault, the benefit payable is an amount equal to the number of Years of Continuous Membership multiplied by $1,000 per year, up to a maximum benefit of $10,000 per Occurrence.  This benefit will not be paid if the Accidental Death Benefit has been paid as a result of the same Accident or Assault.
  4. Occupation / Association Leader Accidental Death & Dismemberment Benefit
    If a Participant's Death is caused by an Accident or Assault which occurs:
    • While the Participant is engaged in any activity which is within the express or implied terms of the Participant's occupation
    • While the Participant is acting in the capacity of an Association Leader, and Death occurs within 90 days of the Accident or Assault, in lieu of the Accidental Death Benefit
    The benefit payable is $50,000.  This benefit is in addition to the Death Benefit.  This benefit will not be paid if the Occupation/Association Leader Dismemberment Benefit has been paid as a result of the same Accident or Assault.
  5. Occupation/Association Leader Accidental Dismemberment Benefit
    If a Participant's Dismemberment is caused by an Accident or an Assault which occurs:
    • While the Participant is engaged in any activity which is within the express or implied terms of the Participant's occupation
    • While the Participant is acting in the capacity of an Association Leader, and Dismemberment occurs within 90 days of the Accident or Assault, in lieu of the Accidental Dismemberment Benefit.
    The benefit payable is $50,000 per Occurrence.  This benefit will not be paid if the Accidental Death Benefit has been paid as a result of the same Accident or Assault.

This benefit is provided by the CTA Economic Benefits Trust, which was established by the CTA Board of Directors to fund the program.

---- Why Do I Need this Benefit?  
There are many incentives to maintaining your CTA membership, and the CTA Death & Dismemberment Plan is certainly no exception.  The CTA Death & Dismemberment Plan provides a life insurance benefit that increases with each year of continuous CTA membership, until the maximum benefit is reached after ten (10) years.  The following table details the benefit levels reached with each year of continuous CTA membership:

The CTA Death & Dismemberment Plan is automatic for eligible members and no premiums are required.
 
CTA members should register for this program and designate a beneficiary by requesting a designation form by

---- How to Use this Benefit  
In the event of a member's Death or Dismemberment, the beneficiary or member should notify:

Death claims must be filed within one year from the date of death.  The Accidental Death & Accidental Dismemberment Benefit or the Occupation/Association Leader Accidental Death & Dismemberment Benefit will not be paid unless a claim is filed within six months after the date of the Accidental loss.
