The CTA Advisory Panel on Endorsed Services, which is comprised of appointed CTA members, reviews complaints that CTA members may have about a CTA endorsed product or service. The Advisory Panel settles complaints and makes binding determinations on CTA endorsed providers. However, issues regarding the credit union or credit card provider are advisory only and are not binding on such providers. The Panel is available at no charge to CTA members participating in CTA endorsed praograms.

How Do I File a Complaint? |

Filing a complaint with the CTA Advisory Panel on Endorsed Services is available at no cost to eligible CTA members.
Become a member of CTA by:
- Requesting an Application at:
(650) 552 - 5278
- You must use the Panel's Complaint Review Request Form. You may download it below, or call (650) 552-5200 and CTA will mail you a form. Please make sure you select the CTA Auto and Home Insurance Complaint Review Request Form, if your complaint concerns auto or home insurance.
- Complete the Request Form and submit it by mail or fax to the CTA Risk Management and Member Benefits Department in Burlingame at least five business days before the Panel meeting, which meets four times a year. Complaints received after the cut-off date will be reviewed at the Panel's next scheduled meeting date, unless special circumstances require an extension of time for processing or review from the carrier.
- The Panel receives all the information that you and the CTA endorsed company have submitted. At the meeting, the Panel will review all the facts, receive testimony, and/or seek clarification and additional information.
- A personal appearance is optional for communicating your concerns directly to the Panel members.
- After all the information is reviewed, the Panel will review the merits of your complaint and make a binding determination either to sustain your position or sustain the company position, or recommend a compromise.
